About us

Community Care Linkages is a strategic initiative of Mass Home Care to link the Massachusetts Aging Services Access Points (ASAPs) network to effectively integrate their services into the evolving healthcare delivery system.

The Massachusetts Aging Service Access Points (ASAPs) are non-profit organizations that have been supporting seniors, individuals with disabilities, and their caregivers in Massachusetts for over 35 years. ASAPs are regionally based and have extensive knowledge of the services and supports available in the communities we serve that support functional independence and healthy aging. Additionally, we have strong ties to local Councils on Aging, Community Health Centers, and faith-based communities to ensure we meet the needs of the consumer and their family caregiver. Together, the ASAPs serve tens of thousands of individuals each year, with satisfaction rates consistently above 95 percent, as measured by standardized surveys required by the Massachusetts Executive Office of Elder Affairs (EOEA).
