IHI STAAR Initiative

The Institute of Healthcare Improvement (IHI) has selected Massachusetts as one of four states for its STate Action on Avoidable Rehospitalizations (STAAR) initiative.  The aim of STAAR is "to reduce rehospitalizations by working across organizational boundaries ... and by engaging payers, state and national stakeholders, patients and families, and caregivers at multiple care sites and clinical interfaces."  The program has 22 major hospitals/academic medical centers and systems participating, and focuses on the collaborative efforts between those hospitals and community resources, such as ASAPs, to meet the cross-continuum needs of the patient.
