Community Care Linkages is a Mass Home Care (www.masshomecare.org) sponsored strategic initiative to link the Massachusetts Aging Services Access Points (ASAPs) network to community-based medical services to enable individuals to live in their homes, at their highest level of functioning possible, for as long as possible. In response to federal and state health reform initiatives, ASAPs are well positioned to integrate their care coordination, information and referral, and care assessment and evaluation services into the evolving care transitions and medical home services under development by physicians, community health centers, hospitals and other medical care providers.
ASAPs are a valued partner in moving health reform efforts forward:
o CCTP/Section 3026 (care transitions)
o Pioneer ACOs
o PCMHs/Physician Practices
o Self Management Supports/CDSMP
o The Community Living Program: Partnering with SCOs, ACOs, PCMHs and One Care Plans to serve dual eligibles (launched Oct 1, 2013).
